Join Our Inpatient Adult Rehabilitation Team at UPMC Northwest!
The 9-bed Adult Rehabilitation Unit offers intensive therapy and 24/7 nursing care to help patients regain their functional abilities. As a Licensed Practical Nurse (LPN), you'll work closely with the healthcare team to provide high-quality care and contribute to patient recovery under the direction of a Registered Nurse.
Responsibilities:
+ Participates in planning, implementing, and evaluating nursing care using focused assessments.
+ Communicates with RNs and healthcare team when care needs exceed the LPN's scope or expertise.
+ Seeks guidance when a patient's condition deteriorates, does not respond to therapy, or requires immediate assistance.
+ Follows established UPMC policies, LPN Practice Guidelines, and State Practice Acts.
+ Performs within LPN practice guidelines, including administering IVs, and contributes to Relationship-Based Care.
+ Develops daily goals and individualized care plans that engage the patient as a partner in their healing process.
+ Documents patient conditions comprehensively to ensure clear communication with the care team.
+ Incorporates professional standards and organizational goals to enhance patient safety, quality, and satisfaction.
+ Provides effective teaching to patients and families for transition to home or other care settings.
+ Actively participates in unit activities, supporting team members and fostering a collaborative environment.
+ Continually seeks professional development and engages in improvement opportunities for patient care.
+ Graduate of a Licensed Practical Nursing Program approved by the State Board of Nursing.
+ 1 year LPN experience on a Rehab Unit or Med/Surg unit strongly preferred
+ IV therapy certification required within 4 months of hire/transfer (automatically included in PA-approved programs post-2012; proof of completion required if prior).
+ Required to complete two annual contributions to improve patient care or hold/maintain a UPMC-approved certification with one annual contribution.
+ Demonstrates age-appropriate care, knowledge of growth and development, and ability to assess and respond to patient needs.
+ Requires physical stamina for patient care duties and proficiency in electronic health recordsLicensure, Certifications, and Clearances:
+ Active LPN license required in the state of practice or a participating Nursing Licensure Compact (NLC) state.
+ If graduated prior to 2012, proof of completion of an approved IV therapy program is required within 4 months of hire/transfer.
+ Basic Life Support (BLS) or CPR certification required.
+ NIH Stroke Scale certification required within 6 months of hire/transfer.
+ Must complete two annual contributions to improve patient care _or_ maintain a UPMC-approved certification and complete one annual contribution.
+ Act 34 and OAPSA clearances required.
+ Employees relocating to the state where the facility is located must obtain local licensure within 60 days of residency change
