Our client is seeking a compassionate and experienced Senior Case Manager to join their dedicated community and social care team in San Jose, California, US . This role is essential in providing comprehensive support and guidance to individuals and families facing complex challenges, helping them navigate resources and achieve their goals. You will be responsible for managing a caseload of clients, conducting thorough assessments of their needs, and developing personalized care plans. This includes connecting clients with appropriate social services, healthcare providers, educational opportunities, and employment resources. You will advocate for clients within various systems, ensuring their rights are protected and their voices are heard. Collaboration with other social workers, community organizations, and government agencies is crucial to provide holistic support. The ideal candidate will possess excellent interviewing, active listening, and empathetic communication skills, coupled with strong organizational abilities to maintain accurate client records and documentation. You will provide crisis intervention, counseling, and ongoing support to clients, helping them build resilience and achieve self-sufficiency. Mentoring and supervising junior case managers, providing training, and contributing to program development will also be key responsibilities. - Master's degree in Social Work, Psychology, Sociology, or a related field.
- Minimum 5 years of experience in case management or social work.
- Valid California LCSW, LMFT, or similar professional license preferred.
- Strong knowledge of social services, community resources, and relevant legislation.
- Excellent assessment, counseling, and advocacy skills.
- Proficiency in case management software and record-keeping.
- Strong interpersonal and communication skills, with the ability to work with diverse populations.
- Demonstrated leadership and mentoring abilities.
