Nurse Practitioner Adult Primary Care (18+)
Salary: $120,000 $130,000 + full benefits
- Active Ohio APRN license (NP)
- National NP certification (Adult-Gerontology preferred; FNP acceptable)
- DEA or ability to obtain
- BLS required (ACLS preferred)
- Reliable transportation
- Ability to work in community-based or non-traditional care settings
Full-time Adult Primary Care Nurse Practitioner opportunity delivering concierge, community-based care to medically complex adult populations in East Cleveland and Euclid, Ohio.
Job OverviewWe are seeking a Nurse Practitioner to provide comprehensive adult primary care and chronic disease management in a high-touch, concierge care model. This role focuses on prevention, medication management, and care coordination for adults 18+.
You will care for patients with chronic conditions such as diabetes, hypertension, COPD, CHF, and CKD while emphasizing patient education, proactive engagement, and reducing avoidable hospitalizations.
This position includes clinic-based care in residential community settings with occasional home visits. The NP will also support care coordination across specialists and lead daily workflows for a small clinical team, ensuring high-quality, patient-centered care.
