Practice Model
* Provide comprehensive patient assessments and individualized care plans
* Manage and coordinate care for post-acute patients in collaboration with interdisciplinary teams

* Participate in a structured 3-month onboarding program with dedicated clinical mentorship
Qualifications
* Active New York medical license and DEA
* Experience in post-acute, acute care, ED, or clinic settings
