Job Description

Trilogy Health Services, a leader in senior living and long-term healthcare, is seeking a dedicated Physical Therapist Assistant to join our Healthcare & Medical Services team. In this role, you will work under the direction and supervision of a licensed Physical Therapist to deliver compassionate, high-quality rehabilitative care to our residents. You will implement individualized treatment plans designed to improve mobility, strength, balance, and functional independence, with a focus on enhancing quality of life for each person we serve.

As a Physical Therapist Assistant with Trilogy, you will provide hands-on therapeutic interventions including therapeutic exercises, gait and balance training, functional mobility training, and use of assistive devices. You will accurately document all treatments, resident responses, and progress in accordance with regulatory and company standards, and communicate regularly with the supervising Physical Therapist, nursing staff, and the interdisciplinary care team to help adjust care plans as needed. You will also help educate residents and their families on home exercise programs, safety strategies, and ways to maintain functional gains.

You will support a safe and encouraging environment by assisting with fall prevention initiatives, proper body mechanics, and equipment set-up and maintenance. You will participate in care conferences, contribute to discharge planning, and help ensure that therapy services are delivered in compliance with state and federal regulations. Working closely with other therapy professionals, nursing, and caregiving teams, you will model Trilogys values of compassion, respect, and dignity in every interaction.
