Physical Therapist Assistant
Job Description
As a Physical Therapist Assistant at Sevita, you will play a key role in helping individuals of all ages and abilities live life to the fullest by supporting the delivery of high-quality, person-centered rehabilitation services. Working under the supervision and direction of a licensed Physical Therapist, you will carry out individualized treatment plans designed to improve mobility, strength, functional independence, and overall quality of life for people receiving home and community-based care.
In this hands-on clinical role, you will provide therapeutic exercises, gait and balance training, functional mobility activities, and use of assistive devices according to each patients established plan of care. You will carefully observe and document patient responses, progress, and safety concerns, reporting changes to the supervising Physical Therapist and collaborating closely on plan-of-care adjustments. You will educate individuals, families, and caregivers on home exercise programs, safe transfer techniques, fall prevention, and strategies to maintain functional gains in everyday environments.
You will travel to homes and community-based settings to deliver therapy services, adapting interventions to real-world environments while maintaining strong infection control and safety practices. You will accurately complete electronic documentation, including daily notes, progress updates, and required regulatory records, in a timely and compliant manner. As part of Sevitas healthcare and medical services team, you will actively collaborate with nurses, physicians, behavioral health professionals, and other rehabilitation team members to support a coordinated, holistic approach to care.
