Remote Senior Social Worker - Clinical Case Management

Our client is a respected non-profit organization dedicated to strengthening communities and providing essential support services. We are seeking a compassionate and experienced Senior Social Worker to lead our clinical case management efforts. This is a fully remote position, allowing you to make a difference from anywhere while collaborating effectively with our team. Our administrative headquarters are in Durham, North Carolina, US , but this role does not require physical presence.

As a Senior Social Worker, you will be responsible for providing high-level case management services to individuals and families facing complex social and emotional challenges. You will conduct comprehensive assessments, develop individualized service plans, and connect clients with appropriate community resources. Your role will involve providing direct support, advocacy, and crisis intervention, as well as mentoring junior social workers and contributing to program development. A strong understanding of social work principles, ethical practices, and community resources is essential.

Key responsibilities include:
  • Conducting comprehensive psychosocial assessments of clients to identify needs and strengths.
  • Developing, implementing, and monitoring individualized service plans in collaboration with clients.
  • Providing direct clinical case management services, including counseling, advocacy, and support.
  • Connecting clients with community resources such as housing assistance, healthcare, employment services, and mental health support.
  • Providing crisis intervention and de-escalation services as needed.
  • Maintaining accurate and confidential client records in accordance with agency policies and legal requirements.
  • Collaborating with interdisciplinary teams, including healthcare providers, educators, and legal professionals.
  • Mentoring and supervising junior social workers and interns.
  • Participating in program development, evaluation, and quality improvement initiatives.
  • Staying current with relevant laws, policies, and best practices in social work.

Required qualifications:
  • Master's degree in Social Work (MSW) from an accredited institution.
  • Current state licensure as a Licensed Clinical Social Worker (LCSW) or equivalent.
  • Minimum of 5 years of post-MSW experience in clinical social work and case management.
  • Strong knowledge of community resources and referral networks.
  • Proficiency in conducting psychosocial assessments and developing service plans.
  • Excellent counseling, advocacy, and crisis intervention skills.
  • Strong organizational skills and attention to detail for record-keeping.
  • Effective communication and interpersonal skills, essential for remote collaboration with clients and team members.
  • Proficiency with electronic health record (EHR) systems and other relevant software.
