Physician Assistant

Signify Health is seeking a Physician Assistant to deliver in-home assessments and value-based care for vulnerable patients. You will perform comprehensive health evaluations, review medications, document in EHRs, and collaborate with interdisciplinary teams to close care gaps. Using modern technology and data-driven insights, you’ll support care coordination, educate patients, and drive improved outcomes. Conduct comprehensive in-home health assessments and physical exams
Review medical histories, medications, and social determinants of health
Document findings accurately in EHR/EMR systems
Identify and close care gaps consistent with value-based care goals
Collaborate with physicians, nurses, and care coordinators on care plans
Educate patients and caregivers on conditions, medications, and preventive care
Use technology and data to inform clinical decision-making
Communicate assessment outcomes to health plan and provider partners
Adhere to clinical quality, compliance, and documentation standards
