Job Description
CVS Health is seeking an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to deliver high-quality, patient-centered care in our Healthcare & Medical Services department. In this role, you will evaluate and treat acute and chronic conditions, prescribe medications, order and interpret diagnostics, and provide preventive and wellness services. You will collaborate with an interdisciplinary team, leverage EHR and telehealth tools, and support population health initiatives. Provide comprehensive primary and episodic care, including assessment, diagnosis, and treatment for diverse patient populations.
Manage chronic conditions using evidence-based guidelines to improve outcomes and reduce avoidable hospitalizations.
Prescribe medications and therapies within scope of practice and applicable state and federal regulations.
Document visits accurately and efficiently in the EHR, ensuring compliance with quality and regulatory standards.
Collaborate with physicians, pharmacists, nurses, and care teams to coordinate integrated, patient-centered care.
Educate patients and families on conditions, medications, lifestyle changes, and preventive health strategies.
Order, interpret, and act on diagnostic tests and screenings to guide clinical decision-making.
Participate in quality improvement initiatives and population health programs aligned with CVS Health goals.
Support telehealth and in-person care models to expand access and convenience for patients.
Contribute to an inclusive, culturally competent care environment aligned with CVS Health values.
Primary care assessment and diagnosis
