This is a rare opportunity to join a great team. + Outpatient + Preferred experience with Diabetes Management + Preferred Certified Diabetes Education or ability to certify + 40 hours per week + Days - Monday to Friday + 8:00am-4:00pm + No weekends or holidays! + Primarily at Christian Hospital, but shared with IL offices as well. North County Endocrinology and GI Associates in located in North St. Louis County. **Preferred Qualifications** **Role Purpose** Provides Diabetes and Nutritional educational services and community resources for inpatients and outpatients. Maintains a case load of patients and promotes program development. **Responsibilities** + Monitors patients response and modifies plan of care accordingly. + Provides accurate documentation as required. + Delivers direct patient care by assessing and evaluating patient's physical, psychosocial and emotional needs. + Develops and implements individual plans of care in accordance with the established guidelines and standards of nursing care. + Educates patients and their families on how to manage their condition. **Minimum Requirements** **Education** + Nursing Diploma/Associate's - Nursing/Nutrition/related **Experience** +
