The Department of Ambulatory Services at Barnes-Jewish Hospital consists of the multidisciplinary staff of BJH Ambulatory Services including responsibility for the GI Labs, Minor Procedure Center, Pain Management Center, Diabetes Center, Resident Clinics, Wound/Ostomy Center and other services as assigned. This department assures that all customers receive the highest level of attention and prompt, courteous service. **Preferred Qualifications** **Role Purpose** Provides Diabetes and Nutritional educational services and community resources for inpatients and outpatients. Maintains a case load of patients and promotes program development. **Responsibilities** + Monitors patients response and modifies plan of care accordingly. + Provides accurate documentation as required. + Delivers direct patient care by assessing and evaluating patient's physical, psychosocial and emotional needs. + Develops and implements individual plans of care in accordance with the established guidelines and standards of nursing care. + Educates patients and their families on how to manage their condition. **Minimum Requirements** **Education** + Nursing Diploma/Associate's - Nursing/Nutrition/related **Experience** +
