Job Description
Hartford Healthcare seeks an OASIS Workflow Coordinator to oversee accurate, timely OASIS documentation within our home health programs. In this role, you will coordinate OASIS workflows, review submissions for accuracy and compliance, and collaborate with clinical teams to resolve documentation issues. You’ll track key quality metrics, provide staff education on OASIS standards, and optimize processes to support regulatory compliance and exceptional patient outcomes. Coordinate and monitor OASIS documentation workflows across home health teams
Review OASIS assessments for accuracy, completeness, and regulatory compliance
Collaborate with clinicians to correct and finalize OASIS submissions
Track and report quality, compliance, and productivity metrics related to OASIS
Educate staff on OASIS guidelines, updates, and best practices
Partner with IT/EMR teams to optimize OASIS-related system workflows
Ensure timely submission of OASIS data to meet payer and regulatory deadlines
Support audits and surveys with accurate OASIS documentation and reports
Identify process gaps and implement improvements to enhance data quality
Maintain current knowledge of Medicare and home health regulations.
