Hartford Healthcare seeks an OASIS Workflow Coordinator to manage and optimize OASIS documentation across our home health services. In this role, you will coordinate intake and ongoing OASIS workflows, review submissions for accuracy and completeness, and ensure compliance with Medicare and regulatory standards. You will collaborate with clinical teams to resolve documentation issues, provide training on OASIS best practices, and monitor quality metrics. Coordinate end-to-end OASIS documentation workflows for home health services
Review OASIS assessments for accuracy, completeness, and regulatory compliance
Collaborate with clinicians to correct and resubmit OASIS documentation as needed
Monitor OASIS-related quality metrics and identify process improvements
Train and support staff on OASIS standards, documentation, and workflow tools
Serve as liaison between clinical, quality, and billing teams regarding OASIS data
Maintain up-to-date knowledge of Medicare and home health regulations
Support audits and internal reviews related to OASIS documentation
Leverage EMR and reporting tools to track workflow efficiency
Contribute to initiatives that enhance patient care quality and outcomes
OASIS assessment and documentation
Knowledge of Medicare home health regulations
Clinical documentation review and auditing
Electronic Medical Record (EMR) systems
Home health operations workflow management
Data analysis and reporting for quality metrics
Process improvement and root cause analysis
