OASIS Workflow Coordinator

Hartford Healthcare seeks an OASIS Workflow Coordinator to oversee the accuracy, timeliness, and completeness of OASIS home health assessments. In this role, you will monitor OASIS documentation, coordinate workflows between field clinicians and coding/QA teams, and ensure compliance with CMS regulations. You will audit records, flag discrepancies, support staff education, and help optimize EMR templates. Collaborating across disciplines, you’ll promote data integrity, drive process improvements, and support high-quality, patient-centered care in a dynamic, team-focused environment.
Coordinate OASIS documentation workflow from intake through final submission
Review OASIS assessments for completeness, accuracy, and regulatory compliance
Collaborate with field clinicians, coding, and QA teams to resolve documentation issues
Conduct regular audits and quality checks on OASIS records and related EMR data
Track and report OASIS quality metrics, trends, and timeliness indicators
Provide feedback and support for staff education on OASIS requirements
Optimize documentation templates and workflows within the EMR system
Ensure adherence to CMS and state home health regulations and internal policies
Escalate high-risk or complex documentation issues to leadership as needed
Participate in process-improvement initiatives to enhance data integrity and patient outcomes
