Job Description

Sevita is seeking a Program Supervisor in our Community & Social Services department. In this role, you will oversee daily program operations, support and coach direct care staff, ensure quality, person-centered services, and maintain compliance with policies and regulations. You will collaborate with families, community partners, and internal teams to develop service plans and coordinate care. Oversee daily operations of community-based health and social service programs to ensure safe, person-centered care.
Supervise, coach, and evaluate direct support professionals, case managers, and related program staff.
Coordinate intake, assessments, and the development and monitoring of individualized service or treatment plans.
Ensure compliance with state and federal regulations, payer requirements, and internal Sevita policies and procedures.
Review and monitor documentation, incident reports, and service notes for accuracy, timeliness, and quality.
Collaborate with families, guardians, clinicians, and community partners to support individuals’ goals and community inclusion.
Assist with recruiting, hiring, onboarding, and ongoing training of program staff.
Develop and manage staff schedules and caseload assignments to maintain adequate coverage and continuity of care.
Support budget oversight, service utilization monitoring, and resource allocation within assigned programs.
Participate in and lead quality improvement initiatives, audits, and corrective action plans.
Respond to emergencies and critical incidents, providing guidance and follow-up to ensure safety and resolution.
Promote and model Sevita’s culture of empathy, inclusion, collaboration, and continuous improvement.
Program coordination and oversight in healthcare or social services
Staff supervision, coaching, and performance management
Case management and service planning
Knowledge of behavioral health, brain injury, or foster care regulations (as applicable)
Understanding of person-centered planning and trauma-informed care principles
Experience with electronic health records or case management systems
Documentation and report writing
Time management and workload prioritization
Budget awareness and resource management
Crisis intervention and de-escalation skills
Verbal and written communication with diverse stakeholders
Basic data analysis and quality improvement methodologies
Knowledge of state and federal regulations relevant to community-based services
Team leadership and interdisciplinary collaboration

Supervisory or leadership training in human services (preferred)
