Program Supervisor

Sevita is seeking a Program Supervisor in our Community & Social Services department. In this role, you will oversee daily program operations, support and coach direct care staff, ensure quality, person-centered services, and maintain compliance with policies and regulations. You will collaborate with families, community partners, and internal teams to develop service plans and coordinate care. Lead and supervise community-based health and social service programs for individuals and families.
Provide oversight to direct-care staff, including scheduling, coaching, and performance feedback.
Ensure person-centered care that aligns with Sevita’s standards, policies, and regulatory requirements.
Coordinate services with families, healthcare providers, and community partners.
Monitor program outcomes, documentation, and compliance using EHR and reporting tools.
Support crisis situations and develop safety and behavior support plans as needed.
Participate in hiring, onboarding, and ongoing training of team members.
Collaborate with leadership to improve services and implement program enhancements.
